Insurance

Please bring your insurance card with you to every visit. This will ensure that our records are kept current and accurate.

If you change your name, phone number, address or insurance plan, please notify us in writing with your signature for verification. You may download and complete our Patient Information form.

Payment

Payment of all coinsurances, copayments, deductibles, and past due bills are payable at time of service.

We accept cash, check, debit, and all major credit cards.

It is important to remember that health insurance coverage varies and not all charges will be covered or paid in full. If your insurance carrier denies a service, you are responsible for the balance of the account. If a claim has been filed with your insurance carrier for over 60 days without a response, please contact your insurance carrier.
